GRANT APPLICATION CHECKLIST

Postmark Deadline: Monday, February 10, 2020

Before mailing, please make sure you have done the following:

__ completed and signed the application form

__ provided a copy of your own 501(c)(3) letter

__ included proof of tax exemption such as 501(c)(3) letter, if you are a city/county municipality or a public school

__ included the 501(c)(3) letter from your umbrella agency, if appropriate, AND the letter authorizing you to use it

__ included a detailed budget of your proposed project

__ included a one-page detailed narrative (including objectives and descriptions) of your program

__ enclosed the original application material PLUS ten (10) copies of all application materials
